Below is the link for ordering DI and PFL applications for your organization or for employers to keep on hand in case they're needed. There are some drop downs that can be confusing, so I provided the drop down responses that should bring you where you need to go to order the applications.
They are free of charge and can be shipped anywhere in California. The Disability Insurance (DI) application is DE 2501, while the Paid Family Leave (PFL) application is DE 2501F. DE 2501FS is the Spanish version of the PFL application.
